Description

Qualifications
- Bachelor's Degree in social work, counseling, psychology, or a related field; or Licensed Social Worker (LSW)
- Minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or minimum of three years' post degree experience working with individuals with severe and persistent mental illness
Responsibilities
- Conduct eligibility evaluations
- Develop person-centered care plans
- Coordinate and authorize program services
