Description
Join Community Passageways as a Recovery Navigator for youth and adults with substance use disorder. Provide critical case management services to connect individuals to community resources and support services.
Qualifications
- 1 year of Case management experience or in a similar position
- Connection and understanding of community resources and systems
Responsibilities
- Engage participants on the street and at social service provider facilities
- Assess participants for severity of chemical dependency, housing status, and mental health needs
- Develop and maintain collaborative relationships with community partners
