Description
Join CareStar, Inc. as a Recovery Manager in the Operations to conduct eligibility evaluations and create person-centered service plans for individuals applying for specific programs.
Qualifications
- Bachelor's Degree in social work, counseling, psychology, or related field
- Licensed Social Worker (LSW)
- Minimum 1 year post degree experience working with diagnosed chronic conditions or minimum 3 years post degree experience with severe and persistent mental illness
Responsibilities
- Conduct eligibility evaluations
- Create person-centered care plans
- Coordinate and authorize recovery services program services
Education
Bachelor's Degree in social work, counseling, psychology or related field; or Licensed Social Worker (LSW)
