Description
Rugged Cross Supportive Housing is seeking a compassionate Case Manager to provide vital support and resources to formerly homeless individuals with special needs in Brooklyn, be a part of something bigger with CAMBA!
Qualifications
- Bachelor's degree
- Experience with SPMI population
Responsibilities
- Conduct initial intake and assessment of clients' needs
- Assist clients in attaining their goals by identifying community resources
- Monitor clients' progress and document via progress notes
Education
Bachelor's degree (e.g., B.A., B.S.W.) and experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience
Working Hours
Full-time (35 hours per week), occasional flexible hours
