Qualifications
- Bachelor's degree in social work, psychology, sociology, or human services
- Experience working with individuals with disabilities or mental illness
Responsibilities
- Provide case management services according to DHS and county requirements
- Maintain accurate documentation
- Develop and maintain positive relationships within the community
Education
