Qualifications
- Master's Degree in Social Work
- Minimum of 1 year geriatric experience
- Experience supporting and/or coordinating community resources and services
- Experience working with geriatric population
Responsibilities
- Provide psychosocial perspective to evaluate and assess Participants' care
- Develop care plans and ongoing services
- Mobilize resources and provide ongoing care management
- Advocate for patient needs and ensure effective care
