Help adults with disabilities live independently by coordinating services with compassion and support.
Qualifications
- BS/BA degree in related course work
- Minimum of 3 years related experience providing case management for individuals with Disabilities
- Experience with Microsoft Office
Responsibilities
- Complete assessments to assist in identifying participant needs
- Develop Person-Centered Service Plan for each participant
- Ensure confidentiality regarding sensitive material and private health information
Education
BS/BA degree in related course work
Working Hours
9am-5pm, Monday-Friday
