Adelante Healthcare is seeking a 340B Medication Adherence Specialist to contribute to value-based care measures, compliance, and medication management for improved patient outcomes.
Qualifications
- High school diploma or GED
- Certified Pharmacy Technician or Certified Medical Assistant
- Three plus years of medication management experience
- Two plus years of healthcare or pharmacy experience
- Experience with 340B preferred
- Strong analytical skills with attention to detail
Responsibilities
- Achieve therapeutic goals and improve patient outcomes
- Assist patients with barriers to care
- Communicate with pharmacy staff and providers on patient outcomes
