Qualifications
- RN or LPN with current licensure to practice professional nursing in the state of Washington
- Bachelor’s degree in Nursing from an accredited program
Responsibilities
- Initiate, screen, and triage all referrals for Yakima County
- Maintain relationships with referral sources
- Complete admission visits to bring patients onto hospice services
Education
Master’s degree in health care preferred
