- 1+ years Hospice Case Manager Experience required
- Current and unrestricted CA RN license

Responsibilities
- Create, implement, and execute overall plan of care for patients in their homes
- Complete initial, comprehensive, and ongoing assessments of patient and family
- Administer medications and treatments as prescribed by physician
