Qualifications
- Graduate of an accredited nursing program
- Current licensed Registered Nurse (RN) in the state of practice
- Experience as an RN in an acute-care setting or Home Health setting
- Valid driver's license and reliable transportation
- Proof of current CPR certification
- Able to pass background check and clearance
Responsibilities
- Provide admission, case management, and follow-up skilled nursing visits for Home Health patients
- Administer ongoing care and case management for each patient
- Coordinate appropriate care with various healthcare personnel
- Prepare appropriate medical documentation on all patients
