Calling all experienced Hospice Case Managers! Join our team in Colorado Springs for a 12-week contract, make a difference, and earn up to $57 per hour. No weekends.
Qualifications
- Minimum of 2 years of experience in hospice care
- Current RN license in good standing
- Strong clinical skills and ability to manage complex care needs
Responsibilities
- Manage and coordinate hospice care for patients in their homes
- Develop and implement individualized care plans
- Provide compassionate support and education to patients and their families
