Qualifications
- 1 year of Primary Care and/or Chronic Disease Management experience
- Current, active, and unrestricted Nursing License in the state of practice
- Strong computer skills, including EMR experience
Responsibilities
- Assist practitioners with patient care and practice specialized nursing skills in patient assessment
- Triage urgent/emergent same-day patients for the team
- Provide support by assisting patients in navigating and coordinating their care
Working Hours
Mon - Fri 8:00am to 4:30pm, Full-Time
