Qualifications
- Diploma, Associate or Bachelor Degree in Nursing
- Minimum one year nursing experience preferred
- Current and unrestricted Registered Nurse licensure
Responsibilities
- Provide admission, case management, and follow-up skilled nursing visits
- Coordinate appropriate care with healthcare personnel
- Prepare appropriate medical documentation on all patients
