Work with EasyHealth, a global community redefining healthcare through technology and compassion. Seeking a Travel Nurse Practitioner to conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members in Wisconsin.
Qualifications
- 2 years of hands-on experience as a Nurse Practitioner
- Must be registered with Medicare and Medicaid
- Current, unrestricted license in the state of assignment
- Must have a valid driver's license
- Must have license/certification: APRN, Certified Nurse Practitioner
Responsibilities
- Complete a detailed evaluation of the medical and social assessment of the beneficiary members
- Identify and refer members/patients for routine, urgent, and emergent case management
- Make recommendations for member's follow-up with their primary care physician
