Exciting opportunity for a Home Health RN Case Manager to join our caring community at CenterWell and provide skilled nursing care for patients in their homes.
Qualifications
- A minimum of one year of nursing experience preferred
- Strong med surg, ICU, ER, acute experience
Responsibilities
- Provide admission, case management, and follow-up skilled nursing visits for home health patients
- Administer ongoing care and case management for each patient
- Coordinate appropriate care with various healthcare personnel
Working Hours
40 hours per week
