Qualifications
- Current Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN)

- Demonstrated knowledge and skills to provide care to and communicate with all ages of the population
Responsibilities
- Providing skilled nursing care to patients in their homes
- Coordinating with the Home Health Team for holistic patient care
- Educating patients and their caregivers on health management
