Qualifications
- Graduate from accredited School of Nursing
- Licensed RN in State
- 1 year experience as professional nurse or baccalaureate degree in nursing
- Compassionate demeanor
- Excellent communication skills
- Excellent assessment and problem-solving skills
- Knowledge of case management processes
Responsibilities
- Plan, coordinate, and provide hospice care
- Collaborate with team members and patients/family members
- Administer prescribed medications
- Evaluate effectiveness of hospice services
- Continuously improve nursing care skills
- Participate in QAPI meetings and activities
- Perform death visits
- Participate in patient's discharge planning process
