Qualifications
- High school diploma/GED required
- Experience supporting families with special healthcare needs required
- Fluency in English and Spanish required
Responsibilities
- Provide care coordination & educate parents/caregivers on navigating care systems
- Conduct family assessments & develop individualized care plans
- Collaborate with service providers & advocate for system changes
Education
High school diploma/GED required; Associates Degree or Bachelor's Degree preferred
