Qualifications
- Current Louisiana LVN license
- 3-5 years of experience as an MDS Coordinator in a skilled nursing facility
- Strong knowledge of MDS 3.0, CAAs, and care planning processes
- Assessment experience required
- Excellent communication, organizational, and analytical skills
- Ability to work effectively in a fast-paced environment
- Basic computer skills, including proficiency in electronic medical records
Responsibilities
- Conduct comprehensive assessments of residents, including MDS 3.0 and CAAs
- Coordinate and develop individualized care plans with interdisciplinary teams
- Ensure accurate and timely completion of MDS assessments, submissions, and corrections
- Maintain records and documentation in accordance with federal and state regulations
- Collaborate with nursing staff, therapists, and physicians to ensure consistent resident care
- Analyze data to identify trends and opportunities for quality improvement
- Participate in survey processes and implement corrective actions as needed
- Stay up-to-date with changing regulations and guidelines
