- Effective organizational and communication skills
Responsibilities
- Identify, coordinate and oversee delivery of services to enhance participants' health and safety
- Develop and monitor service plans
- Provide care management/coordination
Education
Bachelor's degree in Human Service Field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing
