Qualifications
- Bachelor's degree in Social Work or Human Services
- 3 years of Case Management experience
Responsibilities
- Conduct assessments and develop care plans
- Coordinate with NORC Nurse for interdisciplinary care planning
- Maintain accurate records and reports
Education
Bachelor's degree in Social Work or Human Services preferred
Working Hours
Monday-Friday, 9am-5pm
