Exciting opportunity for a Family Care Coordinator-K to provide care coordination services to youth clients with mental health and substance abuse issues and their families at Behavioral Health Care Partners.
Qualifications
- Associate's Degree in Human Services
- QMHS required
- LSW/LPC preferred
Responsibilities
- Provide care coordination services
- Implement client monitoring systems
- Coordinate care with internal and external providers
Education
Associate's Degree in Human Services or related field with one to three years of case management experience
Working Hours
40 hours per week, Monday-Friday
