Description
Looking to join a growing company impacting lives in the community? CareStar, Inc. needs an Assessment Specialist for their Clinical department.
Qualifications
- Active license as RN, LSW, or LISW preferred
- 12 months experience in home and community based services within the past 5 years
- Knowledge of Medicare, Medicaid, home health care and Home and Community-Based waiver programs desirable
Responsibilities
- Conduct program assessments using program and CareStar identified tools and process
- Manage a caseload as needed
Working Hours
up to a maximum of 29 hours per week
