Description
Lead the Patient Care Team at VITAS Healthcare as a proactive Team Manager for various interdisciplinary responsibilities and patient care coordination.
Qualifications
- Bachelor's degree preferred
- Current and valid R.N. License
Responsibilities
- Supervise, evaluate, and coordinate the interdisciplinary team
- Assure continuity of care from admission to discharge
- Serve as patient advocate and coordinator
Education
A bachelor’s degree in a related field is preferred
Working Hours
9am-5pm, Monday-Friday
