  • Board Certified /Board Eligible Internal Medicine or Family Medicine
  • Clear/ Active Medical License in VT
  • Active DEA License
Responsibilities
  • Provide Primary Care services including on-site Behavioral Health providers, Social Workers, and Care Management
  • Diabetes management and education programs
  • Average 16-22 patients a day
