Seeking a Manager of Case Management to lead and coordinate hospital-based case management services, ensuring patient-centered care and optimal resource utilization.
Qualifications
- 2 year / Associate's Degree in Nursing
- 4 year / Bachelor's Degree in Nursing (2-year grace period) OR Master's Degree in Social Work
- 3+ years' experience in Acute-Care Case Management or Social Work
- 2+ years supervisory experience
Responsibilities
- Coordinate all aspects of hospital-based case management
- Supervise and monitor case management team
- Ensure compliance with hospital policies and regulations
